WorkSafe Victoria are giving one lucky fan aged between 5 and 12 years of age the chance to run out with the Western Bulldogs at this Sunday's final round match against Melbourne.

To enter, simply can complete your details on the entry form below and you'll go into the draw to win.

The competition winner will also receive four general admission tickets to the game and two changing rooms passes to watch the players warm-up prior to the match.

A runner-up prize will be awarded, with the winner becoming our KooGa Fan of the Day, receiving a new Western Bulldogs guernsey and appearing on the Etihad Stadium big screen pre-match.

Third prize is two spots in the guard of honour on the field pre-match to cheer the team on as the run out through the banner.

Entries close 11pm Thursday 29th August, 2013 with the competition winners to be contacted Friday morning.

1st Prize: Be our Junior Mascot at Sunday's round 23 match between the Western Bulldogs and Melbourne (one child aged 5-12 years to run out with the team), four general admission tickets and two changing room passes.
2nd Prize: Be our KooGa Fan of the Day, win a new Western Bulldogs guernsey and appear on the Etihad Stadium big screen before Sunday's game against Melbourne, as well as two general admission tickets to the game.
